For a new CAC and/or USID Cards, you are required to present 2 UNEXPIRED forms of ID to your appointment. Approved items are the following: -Valid (Unexpired) Drivers license or ID, Social Security Card, Original copy or certified copy of birth certificate, U.S. Passport or Card
